Sean Kells, 19
In November 1994, Sean Kells was pouring a highly flammable chemical from one ungrounded drum to another when it exploded and ignited. Sean was not told that what he was doing was dangerous and that he should be taking precautions. Sean was killed the third day of his part-time job at the age of 19.
