Tim was severely injured in an explosion two days before
his 21st birthday. Even as he was on fire, he tried to warn
others of the danger. The cause of the explosion was a
design fault in the machine he was operating. There had been similar
incidents, on the same model of the machine, in other cities, but
no one warned Tim. This machine design flaw, coupled with
incorrect placement of the gas-fired hot water heaters in the
facilities room in which he was working, was the recipe for a
disaster. Tim died 10 days later, on April 2, 1996. Only by luck
were there no other serious injuries or victims. |
